Bullzip PDF printer is what I use - freebie. There are others.

Oh, it does, it does .... I once wrote an exemplar distributed file 
system.... the code looked /remarkably/ like the speech recognizer 
I'd been working on the year before. The main difference was the file 
system worked :-)

But back to the the topic,  if the OP is /really/ stuck, I'd 
seriously suggest the postscript (or pdf) intermediate output then 
use psbook and/or its friends to tidy up - the suite (pstools??) is 
quite flexible.
