That's how Tomcat figures out which application should take the request. Where is your application deployed? What WAR file (or exploded-WAR directory)? Any other details that might help explain what's going on? As André said, none of us has a crystal ball (well... one of us does, but he's been MIA for quite a long time). FORM authentication works in Tomcat, whether through an httpd-based proxy or not.
