ok, that was it. there were child pages that were un-hidden. BUT. I thought 
that was the whole point of having pages in a structure, so you could hide a 
whole branch by hiding the parent. Having to go and explicitly hide each and 
every page seems like a bug to me.

If the "Display hidden pages" option from the profile preferences is set to 
"no" and a hidden page is still displayed, then that page might have a 
child which is not hidden.
