After some elimination I found that the stdole32.tlb file is indeed the
key, but that the one from w2k does NOT work.  So, to summarize, I have
wine version 0.0.20031212-1 (debian package). I did the following:

cd
rm -rf .wine
# run winesetup to get a basic wine setup. I used all the defaults.
# (fake windows partition, win98 look and feel)
winesetup
cp stdole32.tlb .wine/fake_windows/Windows/System
# during setup I said ok to install the html help thingy, and it made no
# difference whether I told it to restart or not. In either case, I get
# dumped to the debugger after clicking ok. But the program seems to
# have successfully installed.
wine PAF5EnglishSetup.exe
wine .wine/fake_windows/Program\ Files/FamilySearch/Paf5/paf5.exe
