would this be with the machined sides of the nuts facing down, up, away from
each other, or facing each other. or dose it matter ???

Also, I talked to John Furbur recently and he's got a new twist on the
method shown on the video and on Paul's site: he says to install both nuts
back to back, then the metal washer, then the cap (as opposed to
sandwiching the washer between the nuts). Jacques.
