Not sure if the argumentation here is 100% correct. There are cases where a
CODE_REMOVED event cannot be reconstructed from CODE_ADDED and CODE_MOVED
events. If an entire page is released by the GC then a JitCodeEventHandler
will never be notified about ranges on those pages and might (a) leak space
in internal data structures, or even worse (b) dereference memory ranges
that used to contain code objects but have been returned to the OS.
