I tested 4.0.6 today and found a problem with the USB menu under devices on a running guest.
Host is a Win7 64bit box. >From the settings windows prior to starting the guest, USB device selection Shows an error message stating "Failed to Access USB Subsystem" A screenshot of the error message with details showing is attached to this message. Additionally the USB menu from the devices menu in the toolbar of a running guest does not display anything meaningful. I *think* I see a change in just a few pixels in the top right corner of where the USB devices menu would have appeared if it had worked. This is true no matter if the extension pack is installed or not, and no matter if VBox has admin privs or not. As far as I know, the only fix is a downgrade back to 4.0.4 Thanks for your attention.
