My understand (from having been rooting in the code) is that it looks for blocks of the disk that are all zeros and then removes their data from the VDI file.
The easiest way to get good results would be to create a file full of zero bytes that fills the guest's empty disk space. Then delete the junk file, shut down the VBox, and compact the VDI.
