It means that you have managed to switch from the VM console window to a different application so that the focus_out/deactivation event was not delivered to the VirtualBox application for some reason. We've seen something similar, but so rarely that no chance to find the reason so far. So, if you have a good recipe to reproduce, you're welcome. P.S. When mouse integration is turned on, the mouse pointer is never grabbed (and cannot be). Otherwise, it is grabbed whenever you click inside the VM console window regardless of the current keyboard grab state. However, both the keyboard and the mouse are ungrabbed at once when you press the Host Key (and both will be grabbed again if you press it once more). -- Dmitry A.
