I made a patch to qscanq to block attachments. qscanq is fairly
lightweight, written in C. qscanq calls ripmime which breaks
the email into the file names, for attachments. So it was
relatively easy to check the filenames against a list of
bad attachment names and reject the email, like it rejects
the email if clamd reports a virus.
