well um....
doesn't look like neone actually
hit any thing that would give them a
connection because your clients ip never appeared 
in a pop b4 smtp connection.

your error message, and log extract as listed 
does not show a pop connection cleanring the tcpserver
to clear the ip as an allowed relay.

so your qmail acted normaily and denied a relay status for 
the originating connecting ip.
