We do the time out.  It's documented on the back of the consent form.  We don't 
do the site marking unless there is a reason why that site, and only that site, 
can be used for access.  Otherwise, the "right site" is the one the inserter 
chooses based on assessment.

We have been having discussions as of late at our facility regarding marking 
PICC, CVC sites prior to procedures and also doing a "time out" for these 
insertion procedures.  Does anyone out there do this for these type of 
procedures?
