1) yes, depending on each case, and there are
different levels of sedation.
2) minimal sedation, moderation sedation (supervised
by sedation attending), deep sedation (usually happens
in OR or ICU).
3) minimal sedation -- residents write the orders.
    moderate sedation -- the sedation MD writes the
meds orders.
    deep sedation -- in OR, the anesthesiologist
administers and monitor the whole procedure. in ICUs,
the attending & fellow take care of the meds.
4) depends on level of sedation. my guess is no meds
given at home. you want to give quick onset & short
duration meds. What is the rationales giving the meds
at home?
