There's no cut off recommended.  At our facility some of our nurses
have asked over and over for some concrete numbers.  Can't do it. The
inserter should be experienced and access only the basilic or cephalic
vein if the INR is high or platelets low.  Take some extra gauze.

If a patient needs central access, a Picc is the safest thing to do. I've never given it too much consideration, other than a glance at the
results on the chart and the certainty that this is what the patient
needs.
