We have had this same problem for over a year. There has been no policy from
radiology, nursing or the I.V. Team. I have taken matters into my own hands. I
insist that an order be written that states where the I.V. must be placed, the
name of the test (usually CTPA) and (most importantly)that it be removed when
the Pt. returns to the floor. Basically, it's only to be used as a procedural
catheter.
When there is not an appropriate vein to place a 20ga. angiocath. I write a
note in the chart stating that I'm unable to place the catheter and that the
Pt. is a poor candidate for such a catheter. I also suggest that an alternative
method of venous access be obtained for the test.
This gets me off the hook, but some Pt.'s still must under go the wrath of an
intern or resident who feels their skills and expertise are above mine.

Same problem here.  Some of the problem alleviated with power PICC, but still a very big problem.  They just do not understand that the Infusion Nurses cannot make a vein materialize on some patients.  Very frustrating!

In our hospital we are getting the same requests. We have no choice but to place a line when possible. As we all know there are some pts for whom we can not do this. So far we have not been able to open a constructive dialogue with radiolog about this issue.
