The Sherlock Tip Location System IFU is your best source for information as the IFU has been approved by the FDA and is therefore covered by extensive testing.  Here are the two statements from the IFU that you should know:
 
"The Sherlock detector can be used on pateints who have implanted cardiac rhythm devices such as pacemakers and defribillators.  When a cardiac rhythm device is present, it is recommended that the Sherlock tip location system stylet be placed on the contralateral side."
 
"Temporary disruption of the cardiac rhythm device may occur is the Sherlock tip location stylet passes within 1cm of the cardiac rhythm device.  Use care if placing the Sherlock tip location stylet on the same side as the cardiac rhythm device."
 
Keep in mind, we are speaking of the actual pacer or defib box, not the pacer wires.  So first choice is to place the PICC on the contralateral side.  If this can't be avoided, check to see where the pacer box is.  If it is within 1cm of the subclavian, you should consult the patient's cardiologist before placing a PICC while using Sherlock.  It is also a good idea to have this patient on an EKG monitor during the procedure.  Bottom line, if the Sherlock does affect the pacer--and that's a big if because the magnetic signal put out by Sherlock is so minor that if it does affect the pacer, it will only be a temporary disruption that will clear once the catheter has moved away from the pacer box.

Question for any of you who have used the Sherlock with Bard.  I was told that the Sherlock was NOT contraindicated for use when a patient has a pace maker or electrical device of some kind in their chest.  I have recently been told that you must be careful not to get the device (the Sherlock) anywhere near the implant, and as long as you do not, you are ok to use it.  Does anyone have the actual manufacturer recommendation?  I hate to give out incorrect information.  Paul?  Are you out there?  Kathy?  Do you know?
