If you are talking intraluminal clots, I have seen tpa working for several days old clots. You might have to let it dwell longer.
That would not be true for intravascular clots, as they eventually become one with the vein wall.

How long after clot formation can tpa be used with reasonable success?  I had a case today where 2 lumens of a triple arrow had been clotted for about 5 days.
