I've recieved a few flies that were tied with the hook up and never considered how the fly would be received by the fish ,as I've also, never fished these patterns. It certainly makes sense that there are more critical sites in the upper cavity than the lower.
As to circle hooks, I just got back from Alaska fishing for halibut where we were using circle hooks. The fish seemed to be able to get off the hook quite easily if you didn't maintain a tight line. They are not easy to extract, as I got fairly miffed at the inexperienced deckhand ,who was mortally wounding the small halibut in trying to extract the hook.
