As you begin to make the twists, you will have to hold the running end of the mono between your little finger and palm of each hand.
 
As you begin twisting the tag end of the mono in your left hand around the running end of the mono in your right hand, you may begin to run short of the tag end of the mono.  No problem.  Simply pull a little more mono through the thumb and finger of your left hand and continue.
 
When you have 8 full twists, you should be at this point:
    - thumb and finger of each hand are holding a tag end and running end of mono.  Between the thumb and finger of each hand is twisted mono - 8 twists.
The little finger of each hand is securing the running end of each piece of mono agains the palm.
 
If this is your first try, your hands are probably a foot apart - far too wide.  No problem.
 
Use the third hand - I use my teeth for this step - pull on one tag end and then on the other while holding TIGHTLY to the runninn end of the mono with little finger against the palm.  This will compress the twists and bring your hands together much closer.
