You should now have your hands only inches apart, thumb and finger of each hand holding the mono where the last twist crossed, with 8 twists between your two hands. 
 
Look at what you have.  The tag ends in both hands should be pointing up and the running ends are captured nicely between little finger and palm.
 
This next step is the whole key to the knot.
 
Holding tightly with thumb and finger of each hand, holding tightly with little finger and palm - gently slide the right thumb and finger towards the left thumb and finger which holding tightly to the tag ends between little finger and palm.
 
Here is what should happen.
 
As you slide your thumb, a loop will open in the twisted line between your two hands.  That loop will be approximately in the center of the twists.  1 twist either side does not matter.
 
If the loop is not in the middle, you can compress the twists by pulling gently on the tag end in your right hand with your teeth.
 
Now, thread the tag end of the left side through that loop - thread the tag end of the right side through the same loop but in the opposite direction.  this part will look very familiar to you if you have ever tied a blood knot.
 
Using your teeth, grip the tag ends and gently pull your hands apart.  Gently!  As the knot begins to tighten, you can then moisten the knot and release it from your teeth.
 
Snug the knot securely and clip off the ends.  You can clip each end very closely to the knot.
