Well, I have gone several times.  Like Murf, I guess I have always felt I could go where no man should go and learn that I shouldn't.
 
One memorable time for me was once when I was fishing in the Uintas in Utah in the winter.  My wife dropped me off at a spot and she went up to a dam which is very beautiful in the spring and they wanted to see it in the snow of winter.  Just after they left I got in the water and the current pushed me into a hole -over my head.  They would be gone for a couple of hours and I was soaked.  I was in the mountains with a good amount of snow on the ground.  I had levis on (say stupid) and polar fleece on top.  I swam out of the river, emptied the water from the waders, and stripped.  I wrung out the clothes and put them back on.  I looked around and - well there was nothing.  I thought well the river is probably not any colder than the outside air temperature so I got back in the river and fished until she returned.  It was then I became a polar fleece advocate.  Even wet I stayed warm on top.  Levis well - they are great when dry.  I did catch several fish.  And I started carrying an extra set of clothes whenever I fish.

I guess I've been fortunate, or else the rivers I've waded haven't been too rough, but I've never fallen- yet.  I have fallen getting to the river in some rough country, banging up some bones and gear, though.  Now I choose to float rivers, rather than wade, and I can't wait to try the Mokai out this summer on Wyoming/Montana back-country rivers.
