Its really easy with QT Pro:

Open your movie in QT Pro - Drag the play head to the frame you wish
to set as the thumbnail image - Go to View->Set Poster Frame - Go to
File->Save, and choose to save movie as self contained

I'm pretty sure there is a similar menu option somewhere in iMovie.
