OK, my highly placed insider (seriously!) at Apple podcasting says 
the following:

"Not a great answer here. The QuickTime people tell us that the 
latest  version of Quicktime Pro only creates iPod-playable files 
with the Export to iPod option. They're working on a fix for the 
next version, but for now it's no longer possible to create iPod 
playable video with the h.264 mp4 export option. Frustrating to 
people because the export to iPod option creates larger file sizes."

It doesn't tell us much that we don't know, and doesn't really make 
sense - since an .m4v file that you get with the ipod export option 
is just a h.264 mpeg4 anyway. But there you have it.
