Did you choose to license your videos under the CC license without reading  
it yourself? It's right there in section 4.d. The short version is that  
attribution covers:

  - Your name (or pseudonym)
  - The title of your work (if supplied)
  - The URI ("to the extent reasonably practicable, the Uniform Resource  
Identifier, if any, that Licensor specifies to be associated with the  
Work, unless such URI does not refer to the copyright notice or licensing  
information for the Work") - This is the permalink

If you want very specific attribution like "This video was created by John  
Doe, THE best frog breeder west of the Mississippi. He blogs are  
www.frogs4life.com" then don't use a CC license. CC licenses are designed  
to ce easy to deal with and thus they are broad when it comes to things  
like attribution. Don't like it, don't use it. But don't create extension  
("well, my videos are CC, but really they're CC with my own additions").
