Thanks for all the useful answers. Though I did see the FAQ entry before I posted, I didn't realize any of the solutions there would help me to start the line prefixes at an arbitrary line and do the numbering! However, the tip on using Pandoc as the Vim equalprg not only helps in this case, but opens the door to solutions to many other problems...and that is not a method I would have known to search for beforehand anyway.
