Hi,

Just a few things to check:

Device ID should be zero for the internal HD.

The internal hard drive should have termination enabled.

If it's not an Apple branded hard drive you may need to get a patched 
copy of drive setup to partition/format it. Also, if it is a third party 
hard drive, it may need you to use drive setup in order to install 
appropriate drivers, in which case the OS will not be able to just offer 
to format it for you until you do this.
