I se to have a Mac IIx and the original Apple 24 bit 12 inch display (trinitron tube). I used the Apple stock card but I don't remember the name but it should be documented out there somewhere, maybe on archive.org (waybackmachine).
I would try a different NuBus slot and make sure the cable is not faulty. Also, the display is another part of the equation here so do you know if the display works as expected from another system?
