If you are using VoiceOver, then I'd recommend Meteor Notes for $2.99 or PlainText for free. Both allow unlimited folder creation and can sync with dropbox or email documents. They both use plain text format. Pages is the only app for doing Word Documents, but it is not very accessible with VoiceOver in my biased opinion. It certainly isn't worth $9.99.
