No, You need either to fill one out, online, including addresses and phone of organizations who can certify you are blind or visually impaired to the point that you can not read newspapers or other printed materials, or you can have it sent to you, or you can call 866 504-7300 and speak with the folks who may answer the phone if this does not answer your question.
