It might be good to ask the person what kind of emergencies he is concerned
with.  If he doesn't know, then it might be useful to discuss the kinds of
emergencies he might find himself in and when he would be using the phone.

If there is an emergency, the first priority isn't to call 911.  Instead the
first priority is to seek as much safety as you can in that situation.  I
noticed some situations were mentioned in this chain of e-mails.  For
example, if someone is physically threatening you face-to-face, it's more
important you do what you can to protect yourself physically in that
situation than it is to call 911.  If you need to run, focus on running.
Don't allow yourself to split your focus.  Calling 911 under those
circumstances isn't going to help you.  It's going to take emergency
services a while to get there so you want to make yourself as safe as
possible until they can respond.

Once you have some level of safety, then you can think about dialing 911.
At that point, you should have plenty of time to use whatever method you
want to call 911.

That may be the best way to answer that question for the person.  Just the
thought of emergencies can be scary and a lot of people have unrealistic
expectations of what they can and can't do or what will happen during an
emergency.  The first step is safety and stabilization.  After you do what
you can about that, then you call.  and under those circumstances, you will
have what time you need to call 911 as well as provide them with information
so they can respond in the best way possible.

And regarding noisy environments, I always keep earbuds either in my pocket
or wrapped around my wrist just in case I'm in a really noisy environment
and want to make it easier to hear the iPhone.
