There is a design flaw in how VoiceOver reacts on a telephone call. I think
this may be a quite similar issue to our Belgian friend's problem so have
"piggy-backed" my message with this thread.
A friend complained that her VoiceOver volume was too low during a phone
call and the iPhone was away from her face. I duplicated the problem by
asking Siri to complete a telephone call and, sure enough, now when I'm on a
phone call the VoiceOver volume is much too low when the device is in
Speaker mode.
The problem with this design -- according to Apple engineering this is not a
bug -- is that if you get another call during a conversation it's quite
difficult to understand VoiceOver so the desired decision can be made as
whether to take the second call. It also makes it very tough to execute a
3-way call or to respond to in-call prompts.
The bottom line here is that for those of us who rely on VoiceOver to be
able to interact properly with the phone, we must have VoiceOver speaking at
a clearly audible volume; that volume level is not anything that should be
pre-determined by engineering or product design, but rather should be a
property we as the user of the phone should be permitted to decide. I urge
all of you to file a bug report with Apple on this issue; the accessibility
support engineer handling this problem strongly suggested that we get as
many folks as possible to raise this issue with Apple so there's no question
that it is a problem for we who must work with VoiceOver.

I have a belgian friend that has the following problem: she nolonger has
speech output on her iphone speaker. If she is plugging in ears, all is
working fine and normal. When plugging out the headphone, nothing is heard.
But, supposing she is settingg her timer or alarm clock and the alarm is
ringing, then suddenly voiceover is speaking normally throughout her
speakers. But after that she confirms the alarm with OK to stop, again,
there is no speech output anymore but there is still no problem when she
uses headphones. So I don't believe it's possible that there is a hardware
error because she can play alarm or music sounds.
So What could be wrong?
