The one that really annoys me is pretty recent; Samantha says defeaturing anytime the word defeat is used. It doesn't seem to do this if you move word by word, but if you come across it when VoiceOver is just speaking, as when flicking right/left, Samantha says it the way I described. I'll have to test this to see if it happens with other voices.
