My understanding (I do not have Siri as I have an iPhone 4) is that Siri can set the VO volume down whilst Siri is active. If you give the VO speak all command (two-finger flick up) whilst Siri is active, you then press the volume up button to raise the VO volume. Siri automatically stops listening whilst VO is talking. This is how Apple is able to allow both technologies to work simultaneously.
Note: There are other apps which can cause VO volume to reset to a lower setting. At least with Apple apps, when you adjust the volume back to where you prefer it whilst running the app, the new VO volume setting is then remembered. I have a couple 3rd party apps which require me to turn VO volume up every time I use them because they refuse to remember my VO volume setting.
