Guessing this is a bug: Have you noticed an issue with dictation & replying to a notification? text message as an example? so you will receive a text reply to it ( not opening the messages app) but directly replying from the Notification itself , double tap on dictation . Voiceover will announce whats been dictated and dictation will insert that it's like voiceover + dictation have a mind of there own. Voiceover used to be quiet in prior versions of iOS when using dictation. A temporary workaround for this is once you sure you are ready to dictation turn off VO speech dictate message then resume speech and correct any errors
