I'm getting this on devices that aren't even updated.  What is their issue with 
this?

With fishing scams rampent demanding your id and password for no reason,  why 
don't they say what they're doing and why, so we can 
isolate the fihsing scams from genuine requests.

I also have this problem and it's very irritating. It happens when I power on 
the phone. I never saw this before the 9.1 upgrade.
