The item chooser allows you to search for an item on the screen. It also
allows you to see an alphabetical list of items that you can double tap
to and the VoiceOver cursor will move to that item ready for you to take
action.

I suggest using Voiceover's item chooser. So. In your ringtones
list, do a 2 finger triple tap to bring up the item chooser. There
you know have an index table of all your ringtones.
