When it works correctly the table index allows you to swipe down to the
aphaalphabet need then swipe right to get to the item under that letter.

I suggest using Voiceover's item chooser. So. In your ringtones
list, do a 2 finger triple tap to bring up the item chooser. There
you know have an index table of all your ringtones.
