The reason I keep choosing the OtterBox defender over any other case with a belt clip, is that I haven't yet been able to find a belt clip case that lets me use the phone while it's in the clip. Most belt clip cases require you to put the phone in the clip with the screen facing inward, which, while great for protecting the screen, makes it hard for a VoiceOver user to use the phone while out and about. The only other belt clip case that lets you put the phone in the clip with the screen facing out also has one major flaw, in my oppinion—and that is that while in the clip, the lightning port is obstructed by the clip, making it impossible to charge the phone and have it on your belt at the same time.
