Just thinking out loud here...aside from having "vibrate on ring" & "vibrate on silent" turned on in Settings, sound; Do you have vibrate turned on in Settings,,General, Accessibility? Don't know if turning on vibrate in accessibility will solve your problem, but it is worth checking to see if this might fix the problem that you are having.
