I use Samantha regular on both my iPhone 6S and iPad Air 2, and I don't seem to have these problems. I know I'm lucky and hope I haven't jinxed myself. Maybe I am just not a power user! (smile) The only time I have a problem is sometimes with Blind Fold Solitaire. When I come out of a game the menu won't speak. However, all I do is press the home button and go to the home screen and back into the game's menu and all is well.
I do use Siri, but not all that often. Although when I have used her, I haven't had any problems with voice over coming back. Have a good evening!
