Have you tried rapidly turning down the volume with the side buttons when that 
happens to see if the next time it is not so loud?
Also, I hope you have reported this to Apple. I've noticed that too and have 
reported it.

The idea is to find a reasonable balance between cranking up the volume

Perhaps you could do a recording and put it on dropbox to give an example of
