I'm not sure about the second issue, but to fix where her messages are being 
sent from, go to Settings/Messages/Send and Receive. It's just before the 
SMS/MMS heading. Your daughter should see two things in the resulting screen 
that pops up. One of them will be her iCloud email, and the other will be her 
phone number. She should tap on her phone number to select it, and on her 
iCloud email address to deselect it.
