iOS 10.3 was a major update implementing a new file system and requiring an upgrade to iTunes 12.6..0.1 for compatibility. I don't think the whole OS is downloaded for each upgrade. For example, iOS 10.3.1 which came out about a week after 10.3 was a much smaller download.
