Yes, I called Apple accessibility hotline, they told me to go through the 
"reset all settings" (which caused me to loose an incredible amount of 
data/settings), and it did not help at all!

It happens whenever I use Siri male voice.

Happened on my 5S, and what a scare I got! I had to reset via Home and Powerand 
like described below, the Paulina voice came up. So in the meantime, don't try 
using the Siri voice with Voiceover a ′unless you want to live the experience 
of losing VO funtionality!
