I am in the same boat as you. I get this message every single time I am on the road.
Drives me nuts. Like you, I have it toggled to completely off, and driving mode is manual. I have gone to settings and then to do not disturb and selected activate manually. After I've been in a car I still sometimes see the notification that do not disturb while driving was activated.
