As Richard suggested, just disable the automatic switching in Voiceover Settings > Audio. I don’t agree with the statement that the phone part of the iPhone is the worst part and I would never carry some cheapo flip phone just to make phone calls, this would mean having two cell contracts since I assume these people would still want to have service on the iPhone for texting and so on. I might agree if somebody has physical issues which make it difficult to use the touch screen, but for people who have normal use of their hands I think it’s just practice.
