For things like the time the watch doesn't need any connection. I guess people who don't want to spend the extra $100 and who generally have their iPhone along the cellular option is not important, you also have to remember that if you want your watch on cellular you typically will be charged an extra $10 a month by your carrier so in addition to extra cost to buy it, you pay an extra $120 a year for the convenience of having a cellular connection directly to your watch. Some MVNO's may not even support cellular for an Apple Watch since it is using the e-SIM technology and then again even a cellular Apple Watch is not completely a stand-alone device and a connection to the iPhone is need to set up the watch and for maybe a few other things.
