Hello Hope, To rename a document you have to select Documents, then in the list of documents, place one finger on the one you wish to rename, toggle VoiceOver off, double-tap with that one finger, then toggle VoiceOver back on again. You'll be in an edit field which is the file name. Type the new name, then tap Done. If you've already given the document a name, you'll need to delete the original name or the new one will be appended to it.
