I hope I do not offend with this message. When we start to feel stressed, we often do not do exactly what we believe we are doing. If the problem mainly happens when you are attempting to answer the phone, you may well be becoming more stressed, so are not performing the gesture properly. In other words, you may be touching the display with another finger without realising it, or holding your fingers too close together so iOS is only able to detect one finger. I strongly encourage anyone having complications with commands (iPhone is not doing what I am telling it to do with my commands) really needs to go to settings, general, accessibility, voice-over, practice area, and practice the commands until you do what you believe you are doing, 100% of the time. I even encourage you to feel pressure whilst doing so (imagine the phone is ringing and you need to answer it right now for example). When you do this well, not only do you improve your ability to control your iPhone, you may well figure out what mistakes you were making before.
