I discovered that converting it to ePub and opening it in Voice Dream Reader, all the headings are there, and you can also use its fabulous search feature. The only issue is that it ends up with a title of "unknown" and my last name. I'm trying to sort out which file to edit to get the title and author correct before offering it as well.

I have also taken the liberty of spelling words which are either author's names or unusual names of apps he recommends and/or uses, have describe what the heading levels are used primarily for, and recording it at a good enough level to avoid artifacts because I am not concerned about bandwidth. When the book is done, I'll make it available in either 3gp format, suitable for NLS and Victor Stream playing and the smallest footprint in space on your storage device, or 48K *.mp3s which will be a good compromise for those who absolutely must use *.,mp3 files with the DAISY markup included.
